Lakeland Care is a Wisconsin-based managed care organization (MCO) providing long-term care services and support to eligible elders and individuals with physical, intellectual, or developmental disabilities through Wisconsin's Family Care program. Guided by our mission, we strive to create a world where everyone has the support and opportunities to thrive. Lakeland Care serves as a partner in helping individuals maintain their independence and coordinate necessary care. Additionally, Lakeland Care's subsidiary, Lakeland Care Plus, offers training, coaching, consulting, and IT solutions.
